Activities Aide - - Fairview
Provide service excellence to the residents/clients and staff by assisting in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident's assessment and care plan, and as may be directed by the supervisor.
Responsibilities Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facilityObserve resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated, by noting, reporting, and charting resident behaviorParticipate in community planning related to the interests of the facility and the services and needs of the resident and familyParticipate in discharge planning; development and implementation of activity care plans and resident assessmentsInterview residents or family members to obtain activity informationInvolve the resident/family in planning activity programs when possibleInvolve the resident/family in planning objectives and goals for the residentRefer resident/families to appropriate Social Service personnel when necessaryCoordinate activities with other departments as necessaryKeep abreast of economic conditions/situations and recommend to the Activity Director adjustments in activity programs that assure the continued ability to provide daily activitiesAssist in developing, implementing, and maintaining an ongoing quality assurance program for the activity departmentParticipate in facility surveys (inspections) made by authorized government agencies as necessaryEnsure that all charted activity notes are informative and descriptive of the services provided and indicate the resident's response to the serviceArrange transportation for field trips when necessaryPerform administrative requirements, such as completing necessary forms, reports, etc.
, and submitting such to the Activity Director as requiredDevelop and maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruptionCreate and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facilityReport occupational exposures to blood, body fluids, infectious materials and hazardous chemicals to your supervisorAssist in developing volunteer activity assignments Qualifications High school diploma or equivalent preferredPrior activities experience is helpful, but not essential Wellspring Lutheran Services, Activities, Geriatrics, Skilled Nursing, Fairview .
